We are looking for a qualified Hospice and Home Health RN Case Manager that shares our values and wants to assist in our vision to provide the excellent care that the community expects from us. We pride ourselves on very low employee turnover. We are looking for a qualified RN Case Manager who wants a long term position with a great company. We’re currently looking for a nurse to focus more on hospice but our RN Case Managers generally do both Hospice and Home Health.

Required education:
Associate degree
Required experience:
Nursing: 1 year
Required license or certification:
Registered Nurse (RN)
