Recruit Secure is Hiring a Director of Social Work - Skilled Nursing Home - NY (S) Near Staten Island, NY

Recruit Secure is collaborating with a Skilled Nursing Facility (SNF) located in Staten Island, NY that is seeking a dynamic and energetic individual to take on the full-time role of a Director o f Social Services. The SNF is a long-term nursing and rehabilitation facility with about 300 beds and dedicated to providing highest levels of care and commitment to its residents, their families and their employees. Salary range is dependent on experience with plenty of room for growth. A full benefits package is included.
Responsibilities:
  • Assists in Hiring, Managing and is a resource to the Social Services team.
  • Sets goals for the department and communicates clearly and meets regularly with Social services team
  • Assures that policies and procedures are adhered to for the SNF; makes recommendations, updates and revisions to policies when and if needed.
  • Develops the Social Services plan of care for residents, identifying goals and approaches and participating in inter-department care planning meetings.
  • Participates in Quality processes of the SNF
  • Counseling residents and family members; facilitate family meetings
  • Complete Admissions, Psychosocial Assessments and Advanced Directives and assures placement in medical records.
  • Oversees referrals for health services and facilitates discharge planning process for the department
  • Completing all required documentation for medical records; produces monthly departmental reports for the SNF Administrator.
Requirements:
  • Master's degree in Social Work, LMSW preferred
  • Must have at least 4- 5 years' experience working as a Licensed Social Worker within a Nursing or Rehabilitation Facility.
  • At least 1 years' experience in a Senior or Director role
  • Should have a good working knowledge of available social services and other community resources
  • Must have excellent communication
