Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Duchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full-time Harm Reduction Navigator to support our Coram and Brentwood sites.
Provides recovery support to reduces barriers to care and shares strategies to improve patient health outcomes. This includes educating patients regarding MAT, HCV, risk reduction, medical and medication adherence, and implements programs and materials that serve to promote education on MAT, Naloxone, and treatment.
Essential Functions:
Engages new patients, provides recovery support, overdose support
Refers patients support recovery support services and/or other community resources
Acts as a role model, mentor, advocate and motivator to recovering opioid users in order to help prevent relapse and promote long-term recovery
Ability to share personal recovery experiences and to develop authentic peer-to-peer relationships while maintaining healthy boundaries and ethical standards at all times
Understanding of and compliance with all HIPPA, OASAS, and other health-related legal policies and procedures, as well as internal Sun River Health Center for Recovery policies and procedures
Understanding of and respect for each individual’s unique path to recovery
Description Knowledge of community resources and understanding of community needs
Ability to work with peers, families and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences
Participate in clinical meetings, quality improvement, case conferences, staff meetings, and supervision
Ability to complete electronic and written forms and reports in an accurate and timely manner
Participate in ongoing training, supervision, and professional development opportunities
Provides health education and adherence support to patients
Escort patients to appointments as needed
Facilitate psychoeducational groups
Conduct community presentations on Narcan, substance use, harm reduction, and related topics
Recovery support liaison for regional primary care sites
