Job Title: Memory Care Director MC – Stone Valley

FLSA Status: Salary, Exempt

Salary: $70,000 per year

MINIMUM JOB QUALIFICATIONS AND REQUIREMENTS

  • Bachelor’s degree in therapeutic recreation, social work, or activities related field, preferred.
  • Minimum 2 years’ experience with senior care or assisted living and knowledge of seniors with Alzheimer’s disease or other related dementia preferred.
  • Case management background is preferred.
  • Must have leadership skills.
  • Previous supervisory and/or management experience in a related healthcare setting.
  • Ability to interact with memory-impaired residents and families living with Alzheimer’s or related dementia.
  • Efficient in managing the Electronic Medical Record (EMR), using the required modules to manage health service systems.
  • Knowledge of and ability to conform to applicable laws, rules, and regulations and meets state or provincial health related requirements (as applicable for position)
  • Must have exceptional organizational and customer service skills with the ability to re-prioritize daily.
  • Demonstrated ability to work in a team setting and to convey strong public-relations skills.
  • Experience in computer use and relevant software applications are preferred.
  • Demonstrated ability to communicate effectively (in English), both verbally and in writing, with residents and staff
  • A valid driver’s license and good driving record are required along with proof of auto insurance. May be required to assume the duties and responsibilities of the Community Driver position as needed.
  • Able to work flexible hours and be available to respond to the community during off-duty hours regarding clinical concerns and emergencies.
  • Maintain a positive, respectful, and professional approach with coworkers, residents, and staff.
  • Have effective organizational and project management skills with attention to detail.
  • Ability to work under time constraints and meet department deadlines.
  • Must be at least 18 years of age.
  • Satisfactorily pass Sunshine’s and state regulated criminal record clearance, health screening and TB clearance (if applicable)

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES

Resident Care:

  • Exhibit good judgment, problem-solving and critical thinking skills.
  • Coordinate and facilitate family care conferences in conjunction with updates in the assessment and service.
  • Assist in supporting residents and family during difficult transitional periods throughout the residents stay.
  • Respond to emergency situations when available in the Community to administer first aid when appropriate.
  • Notify management, appropriate resident family member/contact, and primary physician after accidents/emergencies involving the resident, within the required timeframe.
  • Collaborate with all departments to coordinate resident care and ensure communication of resident and staff needs. Lead the Health Services team in an inter-departmental collaborative approach to resident care for seamless person-centered care.
  • Ensure all recording of monthly weights and vital signs and weight changes are acted upon per protocol.
  • Ensure dietary orders and weight gain/loss are communicated to staff and other departments.
  • Report weight gain/loss to Dietician for the quarterly consultation
  • Ensure care staff is providing and documenting services consistent with resident's plan of care in accordance to state regulatory requirements.
  • Ensure nursing acts as the liaison for ancillary health services to include obtaining orders for services; resident visits occur as scheduled and prompt communications between third party providers and team members.
  • Ensure required shift communications and team reporting and documentation occurs.

Health Services Systems:

  • Ensure resident assessments and service plans are completed per protocol in accordance to state regulatory requirements and in a timely fashion.
  • Partner with the Executive Director on changes in care, that result in increased cost for the family.
  • Review medication orders and ensure orders are correctly entered in EMR and delivered to the residents.
  • Monitor the EMR reports to ensure assessments, medications, incident reports, nursing and care staff documentation are appropriately entered into the system.
  • Monitor and oversee the Medication Program and pharmacy performance, including training medication assistants and ensuring they follow State Regulations
  • Ensure the staff is providing and documenting services consistent with the resident care plan.
  • Report resident incidents and any change of condition per company incident reporting protocol to Executive Director and Home Office
  • Ensure the medical records are kept in a manner compliant with relevant policies and procedures and State Regulations, including updating Emergency Face Sheet information.
  • Conduct routine review of the medication program to ensure medications are being stored, delivered and maintained per protocols.
  • Ensure Pharmacy quarterly reviews are conducted, and action plans developed and implemented, as indicated.
  • Ensure residents who have an unusual occurrence are assessed, have the appropriate follow up, required notifications are made, incident reports are completed and entered in EMR
  • Ensure community remains compliant with State Regulations when accepting and retaining residents.
  • Maintain infection control practices within the community and consult with the National Director for suspected outbreaks that may require mandatory reporting.
  • Monitor EMR reports ensuring assessments, medications, incident reports, nursing and care staff documentation are appropriately entered into the system.

Staffing and Leadership

  • Hire, supervise and coach Health Services staff, including Wellness Nurses, Certified Medication Aides/Techs, Caregivers, and any other Health Services staff.
  • Complete required staff performance reviews in a timely fashion
  • Demonstrate effective verbal communications both in one-on-one situations and in meetings.
  • Ensure the staff schedule is completed and posted for staff two weeks before the start of each month.
  • Demonstrate a positive attitude and model effective interpersonal relationships.
  • Assures proper training and certification of all Health Services Team members as required by Sunshine Retirement Living, and state and local guidelines.
  • Utilize effective organizational and project management skills with strong attention to detail.
  • Manage and approve staff time-off requests in a fair and consistent manner.
  • Train and supervise Health Services staff in the correct use of EMR and other company processes.
  • Order and maintain medical supplies, emergency kits and other resident support devices and supplies.
  • Understands and manages within department budget to include labor and expenses and its impact on the bottom line.
  • Able to work flexible hours and be available to respond to the community during off-duty hours regarding clinical concerns and emergencies.
  • Actively participate in the programs and support census building initiatives, event hosting, external and internal program presentations
  • Complete required staff performance reviews, corrective action, and recognition programs
  • Models a safe workplace culture, ensures safety compliance with all staff.

OTHER JOB DUTIES AND RESPONSIBILITIES

  • Performs all work assigned for the agreed-upon salary, accepting no additional payment or non-perishable gifts from residents, vendors, Community Management, or others.
  • Participate in Manager on duty program and rotate on weekends when assigned or needing coverage for staffing.
  • Meet the assigned work requirements of the job. Regular attendance is required.
  • Adhere to dress/appearance code. Changes in attire may be dictated by special occasion or weather conditions. If you have questions, check with your supervisor.
  • Adhere to and follow company policies, procedures, and standards.
  • Promote resident advocacy and demonstrate excellent customer service at all times.
  • Maintain personnel, resident, and business proprietary and/or confidential matters/material in strict confidence with management of the company (and/or other company staff as needed)
  • Ability to keep all business and operations information confidential.
  • Perform other duties as assigned by the supervisor.
  • Maintains current food handlers permit and other certifications as necessary to fulfill state regulations, including minimum annual education requirements to maintain active certification.
  • Serve as a role model for all employees by displaying a responsible, cooperative, and positive attitude.
  • Represent the community in a professional, courteous, and friendly manner.
  • This position has regular supervisory responsibilities over all care staff and all staff in the absence of the Executive Director. HP123
