Monitor common areas to ensure safety in/outside the building and takes corrective action as necessary. Inside- Ensures doorways, stairs and floors are cleared of any obstructions/debris, etc.
Outside – Remove debris/snow to allow building access for client/tenant/emergency personnel.
Conduct full facility walkthrough within first two hours and the last two hours of each shift.
Transportation of clients to appointments, under the pre-approval of the Program Supervisor.
Assist Program Manager/Program Supervisor with apartment visits, inspections/room inspections, client moves, etc, as needed.
Provides crisis intervention as necessary and report incidents/issues to management. Contact EMS/FDNY/Police Department during emergency.
Provides coverage (24/7) for the program as needed.
Overnight staff will enter unit log every ½ hour to ensure client/tenant safety and clearly documents activity.
Provides problem solving intervention to clients on an “as needed” basis.
Assists in overseeing activities of daily living, food preparation for meals, proper hygiene, chores and etc.
Maintains all electronic case records or other required written documentation to standards set forth by St. Vincent’s Hospital Westchester.
Conducts Fire Drills as assigned and accurately document in SQL.
Demonstrates a clear understanding of Medication Management with proper supervision and documentation as per hospital policy and procedures.
Counts all medication on change of shift, completes medication disposal forms, low medication sheet, etc.
Communicates any issues/discrepancies to managers/supervisors immediately.
Competent in completing medication log forms (blue, pink and yellow), storage, uses and sides effects.
Completes and ensures medical consultations are provided to clients prior to medical appointments along with current medication list.
Assist in medication management:
- Supervising self-administering medication
- Ordering refills
- Medication disposal
- Contact client doctor if refills are required
Schedules all needed client medical appointments; including transportation to and from appointments.
Maintain current CPR/AED certifications
Completes maintenance forms in a timely fashion.
Demonstrates basic computer skills, outlook emails and navigate through agency database (SQL).
Attends trainings as assigned ensuring compliance.
Paperwork is processed and filed as mandated by program manager.
Follows hospital dress and hygiene policies.
Ensures proper follow up care is completed.
Maintains cleanliness of the residence as per the Task Checklist.
Complete daily social service logs reflecting all work-related activities performed during each shift, due at the end of each shift.
Adheres to SJMC and departmental policies and procedures.
Additionally, adheres to policies related to Infection Control, Patient Safety, Confidentiality, Patient’s Rights, Advance Directives, Disaster Protocol, Safety and Fire Prevention, Attendance and Punctuality
Completes special tasks as assigned by the management staff/team.
