Description
Essential Functions:
    1. Implementation and coordination of the person-centered program plan (i.e.: ISP, PCSP) to include revisions and updates as needed and identified by the person and/or their circle of support.
    2. Liaison for the circle of support, communication and coordination of all services that the person receives through state and community entities.
    3. Oversight of all services and disciplines as outlined in the person-centered-plan to include DSP, Day/Residential Team Leader, Program Manager, Day & Employment Habilitation Services, Clinicians (OT, PT, SLP, BSP), nursing assessments, adaptive equipment, and environmental.
    4. Ensure that staffing plans and staffing provided are in compliance with the ISP and the needs of the person supported.
    5. Ensure that the person's supported needs are addressed immediately.
    6. Weekly review of staff documentation and data for daily notes, goals, and other necessary provisions as outlined in the person-centered plan. Analyze goal data and notes to assure they reflect the progress toward the program plan goal and update quarterly or as needed per the person supported.
    7. Prepare and distribute scheduled state-regulated reports and upload them to the designated agency database.
    8. Document care coordination notes for each person on the assigned caseload.
    9. Oversight and collaboration of the internal circle of support (DSP, Team Leaders, Program Managers, RN) to include holding meetings on a minimum quarterly schedule and more frequently as needed. Meeting minutes and any training documents will be documented and available in appropriate agency databases.
    10. Complete at minimum one monthly person-centered-site visit (bi-weekly preferred). Each site visit should include monitoring of the quality of services completed by people supported for their satisfaction with the different aspects of their services and life, and suggestions for changes. Monitoring of services should include internal and external circle of support members. Address any unmet needs or grievances of the person supported.
    1. Collaborate with Staffing Coordinator to ensure staff compliance with all acceptable agency policies and local regulations.
    2. Participate in pre-planning for annual meetings and case conference meetings to prepare both required documents and the person-supported with the internal circle of support.
    3. Ensure and protect self-advocacy rights as it pertains to the person-supported services.
    4. Participate in staff new hire orientation and training to provide person-centered orientation for new hires and any ongoing updates as needed.
    1. Cooperate with investigations and audits per state regulations.
    2. Attending and participating in all relevant agency events, workshops, training, and meetings.
    3. In the event that an emergency preparedness or emergency staffing plan is executed, the QIDP will ensure ownership is taken during a state of emergency (Emergency Control Officer)
    4. Ability to acquire preferred provider certifications and training to include (QIDP Certification, Therap Trainer, State Investigator Certification, Positive Behavior Supports, Travel Trainer, SHFT)
    5. Perform other job-related duties as assigned.

  3. Thorough knowledge of program practices and procedures.
  4. Ability to maintain effective working relationships with associates, subordinates, representatives from other agencies, and the general public.
  5. Ability to effectively communicate with individuals from diverse backgrounds.
  6. Ability to effectively supervise, train, evaluate and communicate with program staff.
  7. Ability to research, prepare, review, and maintain reports and documents.
  8. Ability to use sound judgment when making decisions.
  9. Ability to pass a criminal background check; and
  10. Skilled in the operation of relevant computer systems, including hardware and software, and office machines.

Education and Experience:
Requires bachelor's degree in a human service-related field.
  1. Requires one or more years' experience of direct service with adults with intellectual or developmental disabilities, preferably in a community-based residential setting.
  2. Prefer one year's supervisory experience.
  3. Must meet SJCS and funding jurisdiction current requirements for criminal background/records check.
  1. A valid health certificate to meet current funding jurisdiction requirements is required.
  2. A valid state-issued driver's license with a current driving record in good standing is required.
Physical and Environmental Conditions:
Work requires no unusual demand for physical effort.
A work environment involves risks or discomforts, which require special safety precautions e.g., safely operating a motor vehicle, and exposure to aggressive human behavior.
