Job Description
The Home Health Physical Therapist Assistant will administer comprehensive physical therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative physical therapy as prescribed by a physician.

The Home Health Physical Therapist Assistant will administer comprehensive physical therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative physical therapy as prescribed by a physician.
Principal Responsibilities
- Follow plan of care developed by Physical Therapist using a variety of treatment techniques.
- Promote clients' healthy lifestyle by improving strength, flexibility, balance, and coordination
- Consult and practice with other health professionals, involved with patients care.
- Provide quality, personalized, evidence-based care, and proven interventions, specific to patient's needs.
- Encouraging and help motivate the patient during their recovery process.
- Completes required documentation to complete the patients visits and episodes, as required by federal, state and payor guidelines.
- Instructs patients and family/significant others in home exercise programs, to facilitate function.
- Successful completion of a Physical Therapy Assistant program approved by CAPTE, Commission on Accreditation in Physical Therapy Education
- One year of hospital or home care experience is preferred.
- Associate degree in Physical Therapy Assistant
- Must have dependable transportation, possess a valid state driver's license and automobile insurance
- Currently licensed by the Illinois State Board of Physical Therapy.
- Current AHA CPR certification is required.
- Reliable means of transportation, valid IL driver's license and vehicle liability insurance
- Professional behavior and communication skills
- Strong communication skills, flexibility, and professionalism
- Self-direction with a strong sense of team commitment
- Strong organizational and time management
- This job operates in a variety of settings, including but not limited to professional office environment, patient's homes, and car.
- Routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
- Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
- Possible exposure to blood, bodily fluids and infectious diseases
