PurposeTo develop, coordinate and implement programs and services for residents with cognitive impairment; to provide a therapeutic environment that maintains each resident's highest level of well being of mind, body and spirit.Essential FunctionsJob FunctionsServes as charge nurse during shifts worked, when the Director of Resident Care and Administrator are not in the buildingAdministering medications per physician's ordersCommunicating with physicians related to orders, services and care needsFollowing the residents' plans of care as prescribed by the physiciansDocumenting residents services, changes of condition, exceptions and incidents, as well as documentation of all follow-upAssists with developing individualized care plans and interventionsCollaboration with the interdisciplinary team and families to provide care and servicesSupervision of certified nurse assistants and direct care aidesCompliance with state regulations and community standardsCollaboration with external professional health service providersAssists with coordinating the programming of resident care activitiesAny other duties as assigned by the Administrator and/or Director of Resident CareQualificationsMust possess a current unencumbered LPN licenseMust be 21 years of age or olderKnowledgeable of applicable state regulationsMust read, write, speak and understand EnglishKnowledge and experience in working with older adultsAbility to communicate residents and familiesMaintains professional conduct with associates.
