WHAT YOU'LL DO:

As a Resource Specialist, you will organize and implement social services to patients post Emergency Department visit and acute care admission. Within this role, you will receive referrals for individuals from at-risk populations from interdisciplinary team member including physicians, Case Managers, staff nurses and other members of the care team, and will be assigned to a team to follow through acute care continuum. For our team members, this is more than a career — it’s a calling.

BACKGROUND:

• High school diploma required. Healthcare related degree preferred
• Minimum three (3) years in social work, management of resources, or medical field. Clinical experience preferred.
