Summary:
Provide street outreach and Coordinated Entry (CE) assessments to assigned clients, a combination of those living outside and areas not meant for habitation. Target population include those who are living outside and those identified by Police Officers or other community providers as needing services. Work closely with other community-based providers to ensure immediate and long-term health and safety needs of the participants are addressed. Work to engage clients in Case Management and refer to appropriate region and mainstream benefits. Develop/monitor plan to assist in improved recovery, housing status and overall health. Travel throughout the west region is required.

Physical Requirements: Must be able to conduct street outreach which often may include long periods of walking and standing while speaking with clients.
Working Conditions : Frequent travel throughout the Western region of Massachusetts is required. The Coordinating Entry Street Outreach Worker-West will work at multiple office spaces, such as, the Day Center in Framingham and the Satellite office in Franklin, as well as, 7 Bishop Street in Framingham. As part of the responsibilities of this position, the Coordinating Entry Street Outreach Case Worker- West will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services. A successful background check is required.
