Would you like to join our passionate OHH team, supporting people battling addiction and mental health challenges as a Part Time Patient Navigator?

This opportunity is available in the Bangor area. Navigator will see clients in person or via telehealth and be able to work in our Bangor office as required.

Responsibilities:

Patient Navigators on our Opioid Health Home (OHH) team play a vital role in supporting individuals on their path to recovery. The work includes:

• Provide case management services to clients
• Collaboration with OHH team and clients to develop individualized treatment plans.
• Guides client in accessing resources such as General Assistance, medical providers, food pantries, clothing closets, and other services that will support them in their recovery
• Maintain knowledge of community resources
• Ensure clients receive appropriate support and care
• Documenting client progress and maintaining accurate records
• Ability and willingness to travel to see clients

Qualifications

• Experience

at least one year of full-time experience working in a health/social services/behavior health setting in addition to the education requirements below

• Education:

Bachelors or Associates degree or be currently certified as a MHRT/C, LPN, RN, CADC or LADC

• Valid driver's license and reliable transportation
• Familiarity and comfortable in using electronic medical records
• Current Knowledge of state and community-based resources

Job Type: Part-time

Pay: $22.00 - $23.00 per hour

Expected hours: No more than 20 per week
