Position Description

Position Title: Patient Advocate

Department: Refugee Health Services

Status: Full-Time

JOB CONTENT

Position Summary:

  • The Refugee Health Services Patient Advocate works closely with the members of the Refugee Health Team to provide patient and staff support in NFP's Refugee Health Program.
  • The Patient Advocate provides support to all activities surrounding Refugee Health Screening and follow-up, including coordination of health care services for newcomer patients establishing care post-screening.
  • The Patient Advocate provides administrative and patient support to the Associate Medical Director of Refugee Health Services, Refugee Team Nurse and Refugee Health Services Coordinator.

Essential duties and responsibilities which must be performed in order to carry out the position purpose summarized above:

  • Support the initial health screening process and assist the Refugee Health Services Coordinator as needed on screening days.
  • Introduce patients to primary care services, in addition to dental, behavioral health, and pharmacy services offered by NFP
  • Assists with the coordination of specialty care appointments (including TB Clinic) ordered by the provider during health screening, and coordinates transportation with the resettlement agencies and/or health insurance companies as needed.
  • Provides support to care teams once patient establishes primary care, including care coordination and connection to social services/support as needed
  • Communicates all pertinent health information electronically, by fax and/or verbally with case workers at resettlement agencies. Coordinates with case workers as needed to share important information, verify appointments, and any additional updates
  • Communicates with specialists and the TB Clinic as needed for care coordination
  • Support ODJFS data entry and submission of health screening documentation by entering data into the Refugee Web Tool.
  • Participate in community health outreach activities including those targeted at recent newcomers, as well as established individuals and families.
  • Assists with civil surgeon exams for newcomers and other immigrants, the completion of the I-693 form, and coordination of any follow-up care.
  • Provides general administrative support to the Refugee Health Services Team and complete other duties, as assigned

  • Experience with or in a health-care setting preferred
  • Ability to establish and maintain accurate, complete and confidential case files.
  • Attention to detail, ability to multi-task, work effectively as a team member, coach others and follow directions.
  • Fluency in English; fluency in additional language(s) preferred, but not required.
  • Computer proficiency: application of EMR and related functional software.
  • Excellent interpersonal and organizational skills.
  • Ability to establish and maintain effective working relationships with staff, patients and the public.
  • Ability to work independently with minimal supervision.
  • In-depth emotional intelligence: demonstrated self-awareness, accurate self-assessment, sensitivity and empathy, openness, flexibility, reliability and consistency.
  • Personal transportation and valid Ohio drivers' license and car insurance.
