Primary Responsibilities:
Assessment Coordinators generate UDA’s Prior to MDS ARD (with specific emphasis on GG and monitor for timely completion)
oEnsure all UDA’s (nursing assessments) are filled out and locked
oEnsure all UDA’s are done timely
oBIMS (Brief Assessment for Mental Status, interview typically done by Social Services). It is to be open within 7 days leading up to MDS ARD
oPHQ-2-9 (Mood Interview, interview typically done by Social Services) to be opened within seven days leading up to ARD.
oPain Interview- typically done either by Nursing, MDS, or Social Services. To be opened within 5 days leading up to ARD
oPain assessment- typically done by nursing staff
oNorton- typically done by nursing staff
oFall Assessment- typically done by nursing staff
oOral Assessment- typically done by nursing staff
oGG, also called “Functional Abilities and Goals”
This is done by nursing staff in most facilities. GG assessment is used for reimbursement. If we don’t have it, we pay claims back for all payers.
oThe upcoming MDS’s are open in the “in progress list” in PCC to refer to.
