Job Overview The Advanced Practice Provider (APP) Residency is a two-year transition to practice training program with HealthRIGHT 360. During the 2 years of training the residents will be engaged in a diversity of learning opportunities including didactics, preceptorships, and specialty rotations. Residents will start with an intensive boot camp to shore up clinical basics as well as EMR training, an orientation to practice management and team-based care, and basics of Quality Improvement, as well as other topics. They will participate in weekly didactics, begin with shadowing a provider, and slowly begin to build their own practice panel with preceptor support. Specialty rotations will include those that support care for our safety net population, including addiction treatment, mental health treatment, podiatry, HIV care, Hep C screening and treatment, Women’s Health, etc. In the second year of the residency, residents will continue to receive trainings and preceptorship to continue to enhance their clinical practice. Second year residents will also serve as mentors for first year residents. Cohorts of residents will consist of a minimum of two members. The Advanced Practice Provider Resident Director (APPRD) will assume responsibility for design, execution, and evaluation of the structure, content, curricula and evaluation for the APP Residency. Clinically the director supervises, mentors and evaluates the progress of the residents throughout their program. The director will also mentor with other clinical preceptors who work with the residents to create a consistent learning environment. The program areas will include primary medical care services in a safety net community health center setting. Will collaborate with specialists in the rotations which will include but not limited to addiction treatment, HIV/Hep C care, trans care and women’s health., etc. Provides coaching and support for caring for patients in the safety net, particularly those experiencing homelessness, with multiple co-occurring disorders including substance use disorder, mental illness, and multiple chronic medical conditions. In addition the APPRD will have a panel of patients which they will follow in the clinic. The panel will be a percentage of time that allows for completion of residency responsibilities. This is a leadership position and the director will be included in key meetings regarding clinic organization, population health, funding, research and quality improvement. Leads the Residency Advisory Committee and presents regularly on the progress and challenges of the program. Key ResponsibilitiesResidency Responsibilities:The APPRD is an expert primary care nurse practitioner or physician assistant who will blend direct primary care provider responsibilities with clinical leadership and organizational support.Play a leadership role in the recruitment and orientation of residents, curriculum, and evaluation of each component of the program including but not limited to ensuring excellence in didactic presentations, facilitation of case presentations, and providing feedback and coaching to NP residents as well as support for preceptors.Provides and arranges with staff and outside experts to participate in didactic sessions, precepting, and other teaching and coaching for residents as appropriate.Ensure progress of all primary care NP fellows by precepting as well as providing support to specialty preceptors. Reviewing and responding to reflective journals on a rotational basis with other program leaders.Promote the brand, reputation, and model of postgraduate NP training through writing, research, presentation, and policy activities as assigned by HR360, including meeting with funders and potential funders. Coordinate outside electives or trainings which include applications to hospital(s), malpractice coverage confirmation, and evaluation forms. Distribute, collect, and tabulate all evaluation forms regarding residents, rotations and faculty and compiles and generates feedback for appropriate follow-up and reporting for both residents and preceptors.Uses feedback from evaluations to enhance the APP Residency Program.Serves as an information source for the program, institution(s) and community.Active member of the APP Consortium utilizing networking to learn about best practices and enhance the APP Residency Program.Leadership Responsibilities:Coordinates with Deputy Medical Officer (DMO) , Medical Director(s), Administrative Director and leadership to define and implement policies and provide leadership related to APP Residency.Champions and is an active participant in quality improvement activities for the APP Residency and the clinic.Coordinates general orientation of new residents (includes department, clinical areas and institution).Provides direct supervision of the APP Residents and any residency staff, including hiring, evaluation, ongoing supervision, and termination. Clinical Care Responsibilities:Provides excellent direct patient care based upon respective advanced practice role, credentialing, and privileging.Provides thorough, timely, and accurate documentation. Demonstrates competency in all procedures and skills considered essential in their area of practice.Orders, recommends, and interprets tests, treatments, and therapies within their scope of practice.Prescribes medications and non-pharmacologic treatment modalities, as indicated within their scope of practice.Provides expert consultation within practice group and with other members of the interdisciplinary teamFacilitates and promotes evidence based practice that supports patients in their attainment of individualized health goals.In addition to the above responsibilities, other duties may be assigned.Administrative Responsibilities:Maintains compliance with HR360’s policies, procedures, and guidelines for the care of patients to assure care of the highest quality and maintain compliance with HRSA, Managed Care, and other applicable standards.Actively participates in grant writing to support the program.Works with philanthropy and development department to share information regarding the program and successes.Completes all Relias and other required training on schedule. Education and Knowledge, Skills and AbilitiesEducation and Experience Required:Master’s or Doctorate in Nursing (Nurse Practitioner, Clinical Nurse Specialist, Certified Registered Nurse Anesthetist and Certified Nurse Midwife); Master’s or Doctorate in Science (Physician Assistant).Possession of a current, valid California Registered Nurse License; and Possession of a California Nurse Practitioner Certificate; OR Possession of a current, valid California License to practice as a Physician Assistant issued by the California Board of Medical Quality Assurance.BLS certification.Clinical experience with and commitment to working with diverse client populations including the homeless, persons with substance abuse and mental health issues. Experience: 5 years as a Nurse Practitioner, Clinical Nurse Specialist, Certified Registered Nurse Anesthetist, Certified Nurse Midwife, or Physician Assistant. 2 years of experience as an APP lead or manager.Evidence of progressive leadership experience.Minimum 2 years of experience with APP residency as a preceptor or other equivalent academic teaching experience.Knowledge and Skills Required:Culturally competent and able to work with a diverse population.Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency.Desired:Experience with EPIC EMR.Proficiency with Microsoft Office applications, specifically Word Outlook and Teams.Bilingual in Spanish or other Medicaid threshold language. #ZR
