Overview:

We are seeking a dedicated and experienced Case Manager to join our TEAM!

NOTE: This is NOT a remote position

It is 100% in office at The Recovery Village Palm Beach

Lake Worth, FL.

Part-Time position

Monday through Friday

12:30pm to 5:30pm

The Case Manager is responsible for providing case management services to a variety of populations, following applicable State and Federal, and accreditation guidelines.

Responsibilities:

The Case Manager is responsible for providing case management services to a variety of populations, following applicable State and Federal, and accreditation guidelines.

  • Will conduct weekly discharge needs assessments and develop individualized aftercare plans.
  • Will work directly with our patients to make appointments for aftercare including ongoing care in our ARS IOP program, if able.
  • Work directly with referents and payors to coordinate continued care needs. Collaborate aftercare plans with the treatment team. Help clients stay focused on treatment by assisting with employer/legal needs.
Qualifications:
  • Bachelors Degree in health-related field preferred.
  • Minimum two years preferred experience in the medical and/or behavioral healthcare field
