Description

Responsible for providing direct nursing care to clients enrolled in the Adult FSP program according to psychiatric prescriber orders and applicable client care standards, within the LVN scope of practice. The LVN provides a full range of medication support and nursing procedure services such as preparing and administering oral and injectable medications, taking pulse, blood pressure and respiration vitals, providing first aid when indicated and as authorized within the scope of a LVN, and acts as liaison between the psychiatric prescriber, client, pharmacy and members of the client treatment team. DUTIES AND RESPONSIBILITIES:
Functions as a member of a multidisciplinary team. Maintains positive work relationships in a respectful and professional manner. Exercises efficient and effective communication with other staff in order to ensure quality client care.
Performs all duties and responsibilities utilizing a compassion/trauma informed approach in accordance with all Center guidelines, policies and procedures. Demonstrates sensitivity to working with a culturally diverse population.
Monitors and manages oral medications and/or administers injectable medications prescribed by the client’s assigned program affiliated psychiatric prescriber. Monitors for medications prescribed by other physician’s for clients’ physical health conditions including managing for chronic pain and multiple chronic health conditions and monitoring for misuse of prescription medications. Performs medication reconciliation with clients as needed.
Performs basic physical review of clients as needed such as pulse, blood pressure, respiration and head to toe assessments. Provides field based medication support services as needed for those clients unable to present to clinic to receive them. Organizes and supervises the filling of client prescriptions and pillboxes together with clients.
Acts as a point of contact liaison between the psychiatric prescriber, client, pharmacy and members of the client treatment team. Communicates observations and insights to the treatment team and psychiatric prescriber to support and participate in the on-going development of client treatment plans.
Facilitates health education and basic self-care group meetings under the supervision of the clinical supervisor, Director, and/or Medical Director as assigned.

Qualifications


Graduation from an accredited school of licensed vocational nursing or psychiatric technology. Associate degree in nursing preferred.
Current California LVN/LPT license required and must be kept current and in good standing during employment.
Minimum of one-year experience in a mental health treatment setting. Experience working with homeless, serious mental illness and co-occurring populations referred.
First Aid/CPR certification required and must be kept current during employment.
Good driving record and be insurable by the Center’s insurance company and access to a reliable personal vehicle.
Must demonstrate ability to communicate effectively in business English both verbally and in writing. Bi-lingual language capability preferred.
