Sampson Regional Medical Center is hiring a RN Case Manager to join the Case Management team!
Description:
This is a full-time, exempt position working at least three 12-hour shifts (7:00am to 7:30pm) a week including rotating weekends. The selected applicant will act as a liaison between physicians, nurses, ancillary departments, community agencies, third party payers, the patient and family in assessing needs and formulating discharge plans to facilitate continuity of care at the appropriate level. Additional responsibilities include: assist with implementation of discharge plans for hospital patients of all age categories, while not losing sight of the patient's physical and psychosocial needs, to help sick and well persons find the best solutions to their health related problems at the most cost effective price; responsible for daily Cortex screening designed to validate the eligibility for payment by third parties of all care rendered within Sampson Regional Medical Center including obstetrics; obtain pre-certification and re-certifications for inpatient/observation admissions as needed; assist the Social Worker with Discharge Planning when needed; participates as an active member of the Interdisciplinary Care Plan Team to discuss patient and family needs; prepares and presents the weekly UR report to the CEO and Senior Vice Presidents in the absence of the Director.
Requirements:
* Graduation from an accredited school of nursing and licensed by the North Carolina Board of Nursing as a Registered Nurse.
* Bachelor of Science in Nursing preferred.
* Employment in an acute care setting for at least two (2) years.
* Knowledge of the patient referral system and Home Health programs, DRG's, Quality Review and Third Party Reimbursement.
