RN Case Manager with hospital or hospice experience preferred

A DON type position with Case Manager duties and role

This position requires confidence, independent action, initiative, a sense of urgency, and the ability to make decisions and take responsibility for them. A well-suited candidate can react and adjust quickly to changing conditions and come up with practical ideas for dealing with them. The responsibilities are broad in scope, encompassing a wide variety of activities requiring rapid shifts in priorities. This person must be and enjoy the following:

  • A faster-than-average pace; ability to learn quickly, thoroughly, and in detail
  • Strategic, big-picture thinking backed by an added focus on the tactical aspects of the work Setting and achieving specific standards, an emphasis on schedule and quality
  • Primarily task focused, with collaboration required occasionally
  • Independent decision maker who follows physician orders and plan of care, while maintaining nursing scope of practice
  • Ability to follow up closely and critically for standards and schedule
  • Capable of delegating and teaching to other(s)
  • Seeking Registered Nurses & Licensed Vocational Nurses. Job duties include but are not limited to:
  • Focus on the medical needs and treatment of patients in their home according to the active plan of care, as given by the physician and other appropriate personnel
  • Patient assessments
  • Administration of prescribed medication, treatments, and therapies
  • Coordination of care
  • Health promotion and teaching
  • Training of family members

Qualifications:

  • Licensed as an RN or LVN in the state of Texas
  • Minimum 1 year experience, preferred 2 year experience
  • Clean background and criminal record
  • Current Cardiopulmonary Resuscitation Certification (CPR)
  • Ability to work independently and in collaboration with Director of Nurses
  • Understands regulatory compliance with state and federal regulations, The Joint Commission preferred
  • Physical ability to walk, lift, bend, kneel, see, hear, and speak clearly
