*** $5000 for Full Time $2500 for Part Time SIGN ON BONUS***

Compensation is paid on a Per Type of Visit basis - Up to $130.00 per visit

Mileage Reimbursement from patient home to patient home

Qualifications:

• Graduate of an American Physical Therapy Association (APTA) approved program
• Current license from the state Board of Physical Therapy Examiners
• One (1) year home health and clinical experience preferred.
