The Care Coordinator directs and organizes wound care services in collaboration with the Medical Director and Vice President of Operations for Restore First Health in accordance with applicable federal, state, regulatory, and company standards to ensure the delivery of quality patient care is rendered at all times, and that the Plan of Care and physician orders are followed with regard to established clinical protocols.This position functions independently, demonstrates above average communication skills, personal integrity, and works effectively with patients, family members, physicians, staff, and outside agencies as required.The Care Coordinator plays an important role in the implementation and compliance of Restore First Health’s Mobile Advanced Wound Therapy Program’s Policies and Procedures, in conjunction with the national standardization process, to include patient care protocols, documentation tools, wound care formulary and referral criteria. As a Care Coordinator of the Advanced Wound Therapy Program, this position directs clinical team members on planning and scheduling of wound care visits to include wound team travel route planning, medical records, ordering and inventorying amniotic allografts, tracking and logging of tissue IDs, and close coordination with wound care patients, their referring physicians, and various healthcare entities.Job Qualifications
  • Excellent organizational and prioritizing skills required
  • Effective and professional verbal, and written communication skills
  • Nursing Degree from an accredited college or university, or is a graduate of an approved RN or LPN program.
  • Active RN or LPN license to practice in Georgia required
  • Strong attention to detail
  • Excellent organization and time-management skills
  • Ability to speak effectively before groups of customers or associates of the organization.
Job ResponsibilitiesCare Coordination, Quality Assurance, and Scheduling
  • Performs intake on all referrals upon receiving them
  • Schedules referrals for consultation and/or assessment on patients with pressure ulcers and chronic wounds.
  • Schedule weekly patient schedules and design appropriate driving routes to maximize time for patient care. 
  • Reviews charts of patients with wounds. Ensures that wound assessments, care plans, and treatments are clearly and correctly documented and that appropriate wound related treatments are being provided. Investigates all cases with adverse events related to wounds. 
  • Participates in clinical outcome monitoring, follow-up and agency performance improvement initiatives. 
  • In consultation with the Medical Director, reviews and develops with all involved disciplines plan for discharge. 
  • This position requires continuous updating of patient care and facilitating physician orders, medical records transfer and procuring of patient records, and other care coordination as required.
  • The Wound Care Coordinator is responsible for working closely with our Authorization Specialist to confirm coverage benefits and prior authorizations are obtained prior to services being rendered. 
  • In collaboration with the Medical Director and RFH Leadership, implements and executes standards of care, policies and procedures, competencies, compliance and regulatory requirements, assists with fiscal management and quality assurance programs.
  • In consultation with the Medical Director and wound care providers, assists in the development of appropriate plans of care for comprehensive wound management.
Patient and Provider Education
  • The position supports Restore First Health’s Advanced Wound Therapy program education efforts, as well as educates patients and family members on Advanced Wound Therapy and its interplay with current conventional wound care.
  • Acts as primary consultant to the Medical Director and wound care providers. Serves as a consultant to facility staff and advocates with physicians; through training and support, and enables clinical staff to effectively assess wounds, recommend appropriate protocols, and initiate plans of care. 
  • Educates staff on products available for use in the program (in-service staff PRN). Educates families on care of patient(s), and potential referring physicians. 
  • Works closely with the Wound Care Specialists to educate and report updates with respect to intake procedures, medical records transfer, and schedule availability for patients. 
  • Involved in marketing the wound care program through outcomes and relationships with physicians and other various healthcare agencies such as home health companies, skilled nursing, and assisted nursing facilities.
