Description/Job Summary

HEART Services is seeking a full-time Care Coordinator. The Care Coordinator is an administrative position responsible for collaboratively and actively assisting and supporting the participants of HEART Services with organizning resident care, assisting with the development and implemenatation of Recovery Service Plans, and working with residents and their support teams in the development of their care in the areas of: Life Skills, Medical Care, Psyschiatric care, Income, Health Insurance, etc. The Care Coordinator assists the participants of HEART Services attain overall wellness using strength based clinical model as tools for effective engagement.

Responsibilities/Duties

The primary responsibilities of the Care Coordinator are: 

  • Becoming familiar with all regulations of the CRR program.
  • Building and enhancing relationships within RHD and outside providers to coordinate care for residents of HEART Services
  • Through daily, weekly and monthly review, be responsible for analyzing each residents' coordination of care and ensure that all resident charts are up-to-date
  • Provide assistance to residents with the development of recovery goals 
  • Consistently review compliance of individual recovery plans for each resident accessing two or more services across settings
  • Assist all residents with linakage to care and other available resources related to their care
  • Assist residents with developing comprehensive treatment plans 
  • Review compliance of recovery plans with support teams as needed
  • Facilitate routine comprehensive treatment team collaborative planning meetings
  • Track and maintain treatment plan compliance for residents in care
  • Update/respond to weekly and monthly reporting requirements
  • Participates in all related trainings as assigned by Program Director
  • Work with LPN and Program Director to ensure the continuity of care for residents at HEART Services
  • Complete all administrative documentation as assigned.

Required Experience

Two years experience working with Adult Residential Facility, Case Management, Behavior Health, and various medical necessities.

Required Education

Bachelor's Degree in the Human Service field

One year experience working with children and/or adults with Mental Health/IDD/Addiction diagnosis

Required Qualifications

A valid driver's license is required.

Program Summary

HEART Services is a licensed CRR, (Community Rehabilitation Residence), residential program that utilizes evidence based person-driven practices to provide services needed for individuals to maintain independence in the community.

RHD employees and programs in the City of Philadelphia are required to follow the COVID vaccine mandate. Revised PDPH guidance allows for new hires to be partially vaccinated at the time of hire. This means they have received at least one shot in a two-dose series or a single dose in a one-dose series before beginning in-person shifts. The final dose must be received within 8 weeks of hire (start date). This currently does not include boosters.
