HEART Services is seeking a full-time Care Coordinator. The Care Coordinator is an administrative position responsible for collaboratively and actively assisting and supporting the participants of HEART Services with organizning resident care, assisting with the development and implemenatation of Recovery Service Plans, and working with residents and their support teams in the development of their care in the areas of: Life Skills, Medical Care, Psyschiatric care, Income, Health Insurance, etc. The Care Coordinator assists the participants of HEART Services attain overall wellness using strength based clinical model as tools for effective engagement.

The primary responsibilities of the Care Coordinator are:
Two years experience working with Adult Residential Facility, Case Management, Behavior Health, and various medical necessities.
Bachelor's Degree in the Human Service field
One year experience working with children and/or adults with Mental Health/IDD/Addiction diagnosis
HEART Services is a licensed CRR, (Community Rehabilitation Residence), residential program that utilizes evidence based person-driven practices to provide services needed for individuals to maintain independence in the community.
RHD employees and programs in the City of Philadelphia are required to follow the COVID vaccine mandate. Revised PDPH guidance allows for new hires to be partially vaccinated at the time of hire. This means they have received at least one shot in a two-dose series or a single dose in a one-dose series before beginning in-person shifts. The final dose must be received within 8 weeks of hire (start date). This currently does not include boosters.
