Position Summary:
The Strive Clinic at ServiceNet located in Holyoke, Massachusetts is looking for a per diem physical therapist to join our team. Our client population is primarily chronic stroke and TBI as a part of the ABI/MFP waiver. Most of our clients that we serve live in ServiceNet group homes, however we also serve members that live in the community with their caregivers. As a physical therapist at our clinic you would be responsible for completing evaluations on new clients, training staff, completing wheelchair assessments, re-evaluating clients as needed completing documentation in a timely manner, and communicating with team members to ensure continuity of care.

Position Responsibilities:
- Completing evaluations on new clients

- Re-evaluating clients as needed completing documentation in a timely manner
- Communicating with team members to ensure continuity of care.
General Hours:
- The clinic is open M-F from 7:30-3:30.
- 8-10 hours/weekly. Hours will vary depending upon need.
Position Qualifications:
- Hold and maintain MA state licensure as a physical therapist
- Maintain current CPR and First Aid certification
- Meet and maintain all health requirements under DPH standards
- Familiar with EMR software
- Meet CORI guidelines
- Valid Drivers License; reliable transportation.
