Program Description :

PHS’ Community Health and Nutrition Access Program helps New York residents obtain free or low-cost health insurance coverage, apply for the Supplemental Nutrition Assistance Program (SNAP, also known as Food Stamps), and through our WIC program, provides nutrition education sessions led by WIC nutritionists on healthy eating and physical activity, breastfeeding support, and eWIC cards to purchase nutritious foods. Our staff members are culturally and linguistically competent and identify with the communities they serve. They help clients navigate through what can be a complicated and confusing application process, step by step. Each year, we help over 22,000 people apply for or renew free or low-cost health insurance coverage, 14,000 people to apply for SNAP, and 35,000 eligible pregnant and nursing women and children up to five years of age to enroll in the New York State WIC (Women Infants and Children) Program.

Position Summary: The Quality Assurance (QA)/Program Coordinator will conduct Quality Assurance and program coordination activities for the ABD program in Westchester and Rockland counties, including providing general support to the Sr. Program Manager. The QA/Program Coordinator will review and approve enrollment applications on a timely basis, track enrollment errors and returned applications, and assist with data entry. The QA/Program Coordinator will act as a resource to Managers/Directors and serve as the primary on-site contact for 3 Facilitated Enrollers (FEs) based in Westchester and Rockland Counties. S/he will conduct site visits, manage application submissions to LDSS offices, and assist the Sr. Program Manager in coordinating and conducting trainings to meet quality standards. The QA/Program Coordinator will work with the ABD coordination team in establishing the presence of enrollment sites in Westchester and Rockland counties. S/he will monitor outreach, referral, and enrollment numbers from Westchester and Rockland county sites, including analyzing the data and presenting results to Managers/Directors.

Duties and Responsibilities:

· Conduct Quality Assurance on ABD applications by:

o Monitor all applications submitted from FE in MAPSng daily.

o Reviewing applications for timeliness, completeness, accuracy, compliance and consistency with NYSDOH, LDSS and PHS’s integrity standards.

o Tracking enroller errors and reviewing trends and feedback with enrollment staff on a regular basis and conducting refresher trainings as needed.

o Working with PHS management in resolving discrepancies with submitted applications

o Follow-up with LDSS and FEs to resolve outstanding cases.

o Follow-up and respond to FE questions (email or phone) regarding applications or technical issues. o Follow-up with FEs to ensure all applications are submitted per LDSS and PHS application submission protocols.

· Manage the application submission process by:

o Maintaining a working knowledge of application submission protocols at each LDSS office in Westchester and Rockland counties and ensure applications are submitted according to protocols.

o Travelling to LDSS offices to submit applications on a weekly basis, at minimum, and collect all required documentation according to protocols from each LDSS office.

o Follow-up with LDSS on any discrepancies with the application decisions.

o Maintain & secure all documents related to application submissions according to protocols.

o Ensure all application decisions received from LDSS are entered into the MAPSng system on a weekly basis and update the application status.

o Ensure all application decisions are stored securely, per PHS protocol.

· Provide program coordination for enrollment sites in Westchester and Rockland counties by:

o Conducting regular site visits at all enrollment sites to observe FE activities and systems to ensure that all components of the project are aligned with the New York State Department of Health (NYSDOH) Model Protocol.

o Working with the ABD coordination team to develop outreach materials and conduct outreach presentations to Community-Based Organizations and other organizations/coalitions.

o Assisting the ABD coordination team with building connections with partner organizations.

o Assisting in coordinating and conducting trainings and meetings.

o Supporting Program Managers/Directors with the development of program materials such as desk aids, trainings, and workflows to help staff best assist clients.

o Monitoring outreach, referral, and enrollment numbers from Westchester and Rockland County sites, including analyzing the data and presenting results to Managers/Directors.

o Assisting with information required for reporting as needed.

o Supporting the program’s operations duties as needed, including working with program vendors.

o Uploading and organizing program files on SharePoint sites.

· Attend all mandatory trainings related to the FE-ABD program.

· Attend all ABD related meetings & partner’s meeting.

· Maintain current knowledge of Medicaid and relevant information related to the FE-ABD program. · Monitor LDSS updates for Westchester and Rockland counties for ABD related updates and protocol changes and disseminate information to enrollment staff.

· Review ABD workflow on a regular basis.

· Assist in testing MAPSng system and report any technical issues to Bluemark.

· Maintain and validate all items posted in the Bluemark/ABD worklog.

· Act as the first point of contact for application-related questions for FEs in Westchester and Rockland County sites.

· Attend external meetings and events as needed.

· Assist with other administrative program duties as needed.

· Work successfully in a hybrid format, both in-person and remote work expected.

· Travel throughout Westchester and Rockland counties on a regular basis.

Qualifications:

· Associate degree required. with at least 2 years QA experience/ non-profit program experience preferred.

· Bilingual in English and Spanish preferred. · Experience working with Medicaid (preferred).

· Proficient with Microsoft Office Suite, including Excel.

· Ability to lift and carry 20 lbs.

· Ability to regularly travel throughout Westchester and Rockland counties, and occasional travel to PHS’ Central Office in Manhattan. Must possess and maintain a current, valid driver’s license.
