As a Geriatric Primary Care Nurse Practitioner, you will provide comprehensive primary care services to elderly individuals, focusing on their unique healthcare needs. Conduct comprehensive physical and psychosocial assessments of elderly individuals
Diagnose and treat acute and chronic medical conditions
Prescribe medications and order diagnostic tests
Develop and implement individualized care plans
Provide health education and counseling to patients and their families
Monitor and evaluate patient outcomes, adjusting care plans as necessary
Collaborate with other healthcare providers to ensure coordinated and holistic care
Adhere to regulatory and documentation requirements
Requirements Requirements:
Master's degree in Nursing with specialization as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner
Current unrestricted Nurse Practitioner license in the state
Minimum of 2 years of experience as a Nurse Practitioner in primary care
Experience working with elderly individuals is preferred
Ability to work effectively in a fast-paced environment
Excellent communication and interpersonal skills
Compassionate and patient-centered approach to care
Knowledge of geriatric healthcare and best practices
