Clove Lakes Health Care and Rehabilitation is now hiring a full-time and a per diem Social Worker who is committed to providing the best life and care for our residents. The Social Worker is responsible for providing emotional support for residents and their families, assessing their needs and coordinating community resources. The Social Worker also is an advocate for residents rights and facilitates communication between healthcare providers and ensures a more compassionate and comprehensive care environment. The Social Worker directly reports to the Director of Social Work.
We are located at 25 Fanning St, Staten Island, NY 10314.

SOCIAL WORKER QUALIFICATIONS:
