Position Summary
The HIV Health Educator is responsible for working closely with the PrimeCare clinical team in linking patients to HIV prevention services (such as Pre-Exposure Prophylaxis/PrEP), and effectively engaging patients in HIV treatment and care through resources, referrals, and support. This position will be split between an office/clinic setting and at community-based locations ("in the field").
Duties and Responsibilities
  1. Provide engagement, prevention, and treatment services to patients at risk of HIV Transmission, or those diagnosed with HIV as part of an integrated clinical care team.
  2. Identify patient social determinants of health (SDOH) and use a strengths-based approach to support patients in overcoming barriers to care.
  3. Attend appointments and/or support patients "in the field," as needed to advance care and treatment goals.
  4. Connect with patients by phone, electronic media, or in-person to build positive and respectful patient-centric relationships.
  5. Tailor health education services based on each patient's unique identity needs to reinforce and strengthen treatment goals.
  6. Research local and federal options to ensure all identified patients can access PrEP and proactively ensure patients have consistent and affordable access to medication.
  7. Coordinate and collaborate on positive partnerships with community-based HIV service providers that advance HIV prevention services and treatment in Chicago.
  8. Represent PrimeCare Health at community outreach events as assigned.
  9. Participate in multidisciplinary meetings and promote communication among different staff members, to develop and implement the best approaches to care.
  10. Document services through patients' medical record systems and assigned local databases and maintain the highest level of patient confidentiality that meet all HIPPA regulations.
  11. Attend all relevant internal meetings, complete required trainings, and foster ongoing education opportunities to maintain the most current knowledge on HIV Prevention and Treatment.
  12. Other duties as assigned.
Required Skills or Abilities
  1. Effective, concise verbal, and written with a diverse set of professionals from multiple disciplines.
  2. Demonstrated ability to thrive in a team-based environment.
  3. Ability to thoroughly assess and analyze materials, especially application forms and related documents.
  4. Highly detail-oriented, strong attention to detail, and extremely sensitive to deadlines and timelines.
  5. Maintain appropriate and professional boundaries with staff, trainees, and patients at all times.
  6. Demonstrate respect, appreciation, inclusion, and sensitivity for cultural diversity, gender differences, and sexual orientation of patients and co-workers.
  7. This position requires travel to PrimeCare health centers and/or community-based locations. Ability and willingness to use public transportation and/or own a car will be required for this role.
Physical Demands
  1. Must be able to remain in a stationary position 50% of the time.
  2. Must be able to move around the clinic site 50% of the time.
  3. Consistently operates a computer, computer printer, copy machine, and telephone.
  4. Occasionally position self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.
  5. Must be able to transport from one site to another.
  6. Must be able to cover other shifts as necessary.
Required Knowledge, Experience, or Licensure/Registration
  1. Bachelor's degree in Public Health, Social work or related experience is preferred.
  2. 2 - 4 years of experience in a health care environment, and experience with undeserved patient populations preferred.
  3. Bilingual English/Spanish is preferred.
