Job Overview:
We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team. As a Home Health RN, you will provide comprehensive nursing care to patients in the comfort of their own homes. This is a rewarding opportunity to make a difference in the lives of individuals and families in need.
Responsibilities:
- Conduct initial assessments and develop individualized care plans for patients
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patient progress, adjusting care plans as necessary
- Provide education and support to patients and their families on managing their health conditions
- Collaborate with interdisciplinary healthcare team members to ensure coordinated and effective care
- Document patient assessments, interventions, and outcomes accurately and timely
- Provide training to staff on up to date procedures when needed
- Maintain accurate and up-to-date medical records
Qualifications:
- Valid Registered Nurse (RN) license in good standing in state of operation
- Bachelor’s Degree in nursing or related health and human services field from an accredited college or university OR an Associates Degree in nursing from an accredited two-year program
- Minimum of 1 years of nursing experience, preferably in home health or a related setting
- Experience in Patient Assessment, Dementia care, Acute pain management, Epic, Assisted living, Medical terminology, Diagnostic evaluation, Home care, DME is highly desirable
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Compassionate and empathetic approach to patient care
