GENERAL SUMMARY/ OVERVIEW STATEMENT:Summarize the nature and level of work performed.

As a member of the Addiction Consult Team, the Advanced Practice Nurse will respond to the comprehensive needs of both the individual and family with a substance use disorder. Including evaluation, diagnosis, treatment, education, and management of the substance use disorder and co-occurring medical and psychiatric illnesses. This position is 20 hours/week on Mondays & Tuesdays. Some minimal weekend/holiday coverage is required by all providers. Additional per diem hours are available.

PRINCIPAL DUTIES AND RESPONSIBILITIES:Indicate key areas of responsibility, major job duties, special projects and key objectives for this position.These items should be evaluated throughout the year and included in the written annual evaluation.

1. Clinical Practice

1.1Assesses diagnoses and treats the complex, acute, critical, and chronically ill in-patient for urgent and emergent conditions, related to substance use disorder and associated medical complications.

1.2Performs medically delegated aspects of patient care as agreed upon by nursing and medical administrators according to practice guidelines and under the supervision of the collaborating physician.

1.3Demonstrates critical thinking and diagnostic reasoning skills in clinical decision -making.

1.4Provides expert addiction consultation to other nursing, physician and hospital staff.

1.5Is willing and able to engage with patients in all stages of readiness.

1.6 Serves as a patient and family advocate and facilitates ethical and patient-centered decision making within the Addiction Consult Team and inpatient care teams.

2. Education

2.1.In collaboration with team members, in-patient managers, and clinical leadership applies new technology, nursing theories, evidence based research findings and experiential knowledge to support nursing practice and to promote independent professional nursing practice in the staff nurse group.

2.2.Participates in the activities that contribute to the education of health care professionals including presenting and publishing.

2.3.Facilitates the educational experience and participates in the training of medical residents, medical students, physicians assistants and nurse practitioner students on the Addiction Consult Team

4.Research/Research Utilization

4.1 Participates in and supports research activities that will positively influence in-patient care and patient care outcomes.

4.2 Participates in and facilitates quality assurance and performance improvement activities for evaluation of structure, process, and outcome criteria as it relates to clinical practice.

4.3 Is familiar with current trends in health care and their implication to the treatment of substance use disorder. Including: appropriate pharmacologic and non -pharmacologic treatment modalities as indicated for the management of the substance use disorder and related complications.

5.Clinical Leadership

5.1 Integrates systems thinking, ethical reasoning, and problem solving in identifying interventions that facilitate quality, cost-effective patient care outcomes.

5.2 Identifies factors contributing to outcomes, such as complications, increased length of stay, and over utilization of services and actively participates in problem solving these issues, in collaboration with departmental managers.

5.3 Facilitates patient movement through the system and efficient use of resources through interdepartmental collaboration- this includes the triage of patient consults to the appropriate interdisciplinary members of the Addiction Consult Team.

5.4 Analyzes clinical and non-clinical variables to anticipate patient care needs and predict responses.

5.5 Negotiates conflict to promote collaboration.

6.Professional Development

6.1 Engages in self-performance appraisal on an annual basis, identifying areas of strength as well as areas for professional and practice development.

6.2 Identifies individual learning needs and goals and develops a plan to meet them.

6.3 Maintains clinical knowledge and skills based upon current nursing and health care practices.

6.4 Maintain membership in professional nursing organizations.

6.5 Maintains the standards and educational requirements specific to the nurse practitioner role and licensure.

6.6 Maintains and updates clinical knowledge and skills based on current health care practice/research.

SKILLS/ABILITIES/COMPETENCIES REQUIRED:Must be realistic, objective, measurable and related to essential functions of this job.

Relying on advanced training in psychodynamics, pathophysiology, physical assessment, pharmacology, treatment intervention, addictions management, harm reduction, and relapse prevention, the APRN provides an assessment and assistance with establishing comprehensive plans that address a broad range of concerns for the patient with substance use disorder.

These include but are not limited to:

  • treatment recommendations for intoxication and withdrawal states
  • identifying those with at risk alcohol/substance use
  • assisting with facilitating appropriate discharge dispositions
  • coordinating involuntary treatment options
  • providing education/referrals
  • coordination of services between providers, including others on the ACT team
  • assessment of co-occurring psychiatric diagnosis
  • psychopharmacological recommendations

A qualified candidate must demonstrate strong interpersonal communication, conflict resolution and organizational skills.

Qualifications:

LICENSES, CERTIFICATIONS, and/or REGISTRATIONS (if applicable):Specify minimum credentials and clearly indicate if preferred or required

Current licensure as a Nurse Practitioner or Clinical Nurse Specialist in the Commonwealth of Massachusetts, and Mass Controlled Substances Licensing

Certified Addictions Registered Nurse - Advanced Practice (CARN-AP) (preferred).

Current federal and state controlled substance certification/registration or eligibility.

EDUCATION: Specify minimum education and clearly indicate if preferred or required

Master’s in Nursing

EXPERIENCE: Specify minimum creditable years of experience and clearly indicate if preferred or required

Previous advanced practice experience preferred.

Previous experience working in mental health/substance use disorder care required.

Previous in-patient nursing experience preferred.

Previous experience in a consultative role preferred.

SUPERVISORY RESPONSIBILITY (if applicable):List the number of FTEs supervised.

FISCAL RESPONSIBILITY (if applicable):Indicate financial “scope” information, i.e.: size of budget, volume, revenue, etc.; Indicate total physician/non-physician FTE scope

WORKING CONDITIONS: Describe the conditions in which the work is performed.

Provision of episodic/consultative/recurring care to the adult patients (including geriatric) in MGH in-patient units, who are diagnosed with either primary or co-occurring substance use disorder.

Patients will be seen on many different in-patient units, which requires routine walking to and from various units in the course of a day.Computers and phones utilized on the units are unit-based computers, shared with all other health care providers.
