Description
The essential functions include, but are not limited to the following:
•Reviews and enters screenings, immunizations, and the rest of EHS-require forms into ChildPlus
•Follow-up on incomplete 30-day requirement, 45-day sensory screenings and 90-day health determinations
•Complete referral form when concerns identified on medical documents or intake forms.
•Follow-up with Health Service Manager in regard to health referrals
•Process documentation necessary when requesting medical records of all children in the
•Program
•Conduct initial and /or follow-up calls to medical providers regarding pending medical.
•Records
•Seek, maintain, and update health education materials, health insurance and clinic.
•information.
•Assist with staff trainings.
•Participate in conducting and assisting with self-assessment and community assessment.
•Demonstrate ability to work independently and as an effective member of a
•multidisciplinary team.
•Participate in Early Childhood & Family Engagement - EHS & other LAEP departments in
•staff meetings and trainings.
•Work flexible hours to meet the needs of families (e.g., evenings or Saturdays).
•Perform other duties as assigned
•Assist in planning and organizing the quarterly Health Services Advisory Committee (HSAC) with the community, EHS parents, and staff.
•Facilitate and advertise virtual or in-person health and nutrition education classes to Early Head Start parents and staff based on community needs.
•Collaborate with Early Childhood Educators and Teachers to review missing/expired health paperwork and medical/dental records.
•Partner and collaborate with neighboring organizations to create an effective resource and referral list.
•Meet with families with nutrition and health concerns to create goals and guidance.
Requirements

  • Bachelor of Science in Nutritional Science or relevant certifications (ex. LVN, CAN, or Early Head Start Health Certification)
  • Associate degree in health or nursing education or LVN or 5yrs. work experience, 3yrs. of experience with Early Head Start or Head Start.
