We are looking for full-time motivated and passionate CNA's in St. Croix and Pierce county to provide personal care and related services to assigned patients under the direction and supervision of the patient's case manager. The aide is responsible for documenting all care provided and communicating regularly to other members of the patient care team regarding the patient's status. This position would primarily be working with in a facility caring for patients on our service.
Job Type: Full-time
Salary: $18.00 - $21.00 per hour
Expected hours: 32 - 40 per week

Requirements
Minimum Qualifications

• Satisfactory completion of training program or competency test for Home Health aide which meets the requirements of the State of Wisconsin Department of Health as well as Medicare (CNA).
• Home health and/or hospice experience preferred.
• Current Wisconsin Driver's license.
• Name is listed on the Wisconsin and Minnesota registries as current and eligible to provide Home health and hospice care.
• Excellent communication skills.
• Believes in and has a working knowledge of the hospice philosophy of care.
• Basic Computer skills.

Salary Description

$18 to $21 per hour
