About this position

Under the supervision of the Early Learning Department’s health supervisor, the child and family health specialist will play a vital role in connecting families and children within the Early Learning and School-age programs to health and mental health services. This individual will collaborate closely with families, staff, and community partners to facilitate health-related resources, screenings, referrals, and support. In addition, this individual will be responsible for ongoing monitoring of health and mental health services to ensure the program is compliant with current standards and procedures. The successful candidate must be able to communicate effectively, work both independently and collaboratively within a team, and thrive in a fast-paced environment. What you’ll be doing

  • Collaborating with the health supervisor to design and implement community health outreach programs, initiatives, and events to engage families and children;
  • Building and maintaining strong relationships with families, acting as a point of contact for health-related inquiries, resources, and support;
  • Conducting health screenings, assessments, and referrals for children and families, identifying potential health concerns and connecting families with appropriate services;?
  • Providing personalized health navigation services, guiding families through available health and social service resources;?
  • Collaborating with local organizations and agencies to ensure families have access to a comprehensive range of support services;?
  • Assisting in the coordination of health education workshops and training for families and staff members;
  • Supporting the health supervisor in tracking and maintaining health-related data and records for program evaluation and reporting purposes;
  • Participating in School-age Department activities to ensure a seamless approach to community health and well-being;
  • Attending regular meetings with the health manager to provide updates on outreach efforts, challenges, and successes;
  • Staying informed about relevant health policies, trends, and resources to provide accurate and up-to-date information to families and staff;?
  • Actively participating in committees, such as the Health Services Advisory Committee and the Staff and Wellness Committee, in addition to ongoing training;?
  • Effectively collaborating with other component areas to ensure comprehensive services are provided to children and families;?
  • Following organization and program procedures, policies, and standards;?
  • Accurately completing assigned paperwork and documentation in a timely manner;?
  • Demonstrating professionalism, integrity, and a positive attitude in all interactions with colleagues, families, and within component areas;?
  • Identifying and reporting any suspected incidents of child abuse (following the Mandated Reporter Guidelines);?
  • Communicating with children, families, and other staff in a respectful and culturally sensitive manner;?
  • Maintaining confidentiality of children and family records and files following HIPAA guidelines;
  • Working evenings and weekends and to traveling locally and out of the service area;
  • Performing other duties as assigned by the health supervisor and/or health manager.

What you bring to the table

  • Combination of 12-semester units in nursing, social work, public health (or related field as deemed by the organization) with a minimum grade of “C” (up to six units in child development can be substituted for social work or sociology);?
  • Strong attention to detail and effective organization skills;?
  • Working knowledge of local community resources;
  • Proficient word processing and computer literacy skills, including knowledge of Microsoft programs (Word, Excel, PowerPoint, Teams, etc.);?
  • Ability to communicate in written and oral English correctly and effectively;
  • A California driver’s license, daily use of an automobile, and automobile liability insurance;
  • Command of Spanish, Cantonese, Mandarin, or Vietnamese (preferred).

Job specifications

  • Opportunity for hybrid work-schedule may be available and to be determined by management.
