**Schedule:** Part time, 24 hrs/week, Monday - Friday, varied day/evening schedule, generally 8 am - 4:30 pm. May have to occasionally adjust schedule to meet patient wound care needs. May be combined with part time Diabetes Educator position for full time hours.

**Responsibilities**

Provide direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.

Implement and monitor patient care plans. Monitor, record and communicate patient condition as appropriate.

Serve as a primary coordinator of all disciplines for well-coordinated patient care.

Note and carry out physician and nursing orders.

Assess and coordinate patient's discharge planning needs with members of the healthcare team.

**Requirements**

Licensure / Certification / Registration:

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.

Licensed Registered Nurse credentialed from the Kansas Board of Nursing required.

Certification specializing in Wound Care credentialed from the National Alliance of Wound Care and Ostomy (NAWCCB) preferred.

Education:

Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.

**Additional Preferences**

Bachelor's of Science in Nursing preferred. Wound care experience preferred, but not required. Wound care certification within 12 months of hire.
