Immediate Opening: Urgent Care Nurse Practitioner Needed in Philadelphia, PA
Description: We provide concierge health services using a PCP-driven care model for the most clinically complex and vulnerable community members. Our holistic approach addresses both medical and psychosocial issues to improve outcomes for high utilizers of healthcare services.
Pay Rate: $70-$80 Hourly (paid weekly and eligible for health benefits)
Shift: Monday – Friday (8a – 5p with 1hour lunch)

Job Summary: The same day care provider manages acute illnesses, exacerbations of chronic conditions, and reassures members that ER visits are not always necessary.
