We are looking for experienced LPN’s to work for one of our external clients, a home care agency with locations in all 5 boroughs of New York. The services include residential programs (supervised living, supported living, and independent living), community habilitation, and day habilitation.

This is a permanent, full-time job.

Schedule:

• Monday, Tuesday, Wednesday 7am to 3:30pm
• Thursday and Sunday 11:30am to 8pm

In this role you will be responsible for responsible for providing medical treatment, first aid, emergency care and medications, under the direction of a Registered Nurse.

An ideal candidate would be a graduate of an accredited program of professional nursing, home care experience will be a plus.

Required License/certification:

• License and current registration as a Licensed Practical Nurse in the State of New York
• Valid New York State Driver’s License and maintain it in good standing
• CPR

Responsibilities include, but not limited to:

• Provides health care to individuals receiving services as instructed by registered nurse.
• Provides direct hygiene and health instruction to individuals receiving services and families as required.
• Provides first aid to persons in need of immediate care, with referral to registered nurse or outside emergency services as necessary.
• Interacts with a interdisciplinary team and is responsible for assisting Registered Nurse in coordinating following through on provisions of health care services for assigned residences.
• Keeps progress notes under supervision of registered nurse for each client and summarizes for quarterly, semi-annual and annual reviews.
• Assists physicians in medical examinations as needed.
• Schedules appointments and follows up on physicians’ recommendations.
• Keeps up to date and accurate records of examinations, immunizations, illnesses, accidents and other pertinent data via progress notes.
• Administers medications when appropriate. Supervises storage of medications and assists in oversight and ordering of medications.
