The Nurse Home Visitor is responsible for providing home visiting services to pregnant women, target children and their families that focus on linking pregnant women with prenatal care, promoting strong parent-child attachment and coaching parents on learning activities that improve their target child’s development. The home visitor will provide substance use counseling and services utilizing the Seeking Safety evidence-based counseling model.
Nurse Home Visiting
· Conduct home visits for the purpose of service provision
· Address the social and emotional needs of the children in families affected by substance abuse
· Facilitate referrals to family-centered treatment and recovery services
· Completion/update of the Plan of Safe Care (POSC)
· Participate in staffing and attend supervisory staff meetings, in-service training and professional development sessions as required
· Establish and maintain liaisons with community agencies for member professional development and service opportunities
· Attends participant case staffing as designated
· Collect health related and demographic data during assessments
· Document all contacts with participants and referral agencies in Well Family System
· Attend in-services, conferences, workshops, and community meetings.
· Maintain a courteous and cooperative working relationship with participants, the community referral and partnering agencies.

All other duties as assigned by the Director of Seeking Safety.
