The admissions RN will visit patients in their residence to assess patients home care needs and develop plan of care. Conduct one on one visits with patients. Communicate effectively with patients, caregivers, referrals, and Next 2 Heart staff. Verbal and communication skills. Perform supervisory visits for clients, every 75-90 days
