Neighborcare Health is Hiring a Diabetes Educator - Program Manager III for Registered Dietitian and CDCES team (0.6FTE - 1.0FTE) Near Seattle, WA

The DSMES and MNT Program Manager will guide the Neighborcare Health DSMES and MNT Team to optimize the health status of all Neighborcare Health patients by ensuring patient access to up to date, accurate and patient centered Medical Nutrition Therapy (MNT) and Diabetes Self-Management Education and Support (DSMES). The Quality Manager, DSMES and MNT Program will work with all members of the health care team to promote appropriate and accurate nutrition and diabetes information to help reduce patient barriers to health management. This position consists of 60% direct patient care providing DSMES and MNT and 40% administrative duties providing quality improvement, mentoring, support and MNT/DSMES policy/procedure updates as needed for RDN, CDCES team members, nurses, medical providers, and other members of the health care team.

Key responsibilities:

  • Oversees the planning, implementation, and evaluation of DSMES and MNT services.
  • Coordinates the identification of DSMES and MNT stakeholders and liaises between the MNT/DSMES team members, stakeholders, other departments and Neighborcare Leadership.
  • Monitors and facilitates maintenance of MNT/DSMES team members qualification (CE credits, training, competency, licensures, and registrations).
  • Participate in performance evaluation protocols and policies, as applies.
  • Assures annual learning conversations are completed according to the organization’s evaluation calendar, as applies.
  • Ensures DSMES outcomes are tracked.
  • Ensures ongoing continuous quality improvement projects within the program
  • Completes the DSMES Program Recognition Annual Status Report in a timely manner.
  • Responsible for maintaining DSME Program Recognition and leading the evaluation of the DSMES service’s effectiveness.
  • Maintain professionalism and confidentiality in administrative and patient care areas.
  • Develop and guide the strategic vision of the MNT/DSMES program in collaboration with operations and clinical leadership and in alignment with Neighborcare Health’s organizational strategic goals
  • Analyze education, training, and professional development gaps to determine training needs and learning objectives for staff regarding MNT/DSMES
  • Partner with organization leaders to design and develop training programs and materials to support clinical staff and meet identified objectives regarding MNT/DSMES
  • Implement or delegate implementation of staff trainings regarding MNT/DSMES using evidence-based learning modalities
  • Evaluate the outcomes of trainings and make changes based on feedback from staff and leaders.
  • Develop and implement programs to strengthen career development pipelines with an equity, diversity, and inclusion lens, including systems for recruiting, orientation, training, mentoring, retaining and staff development for RDNs and CDCES.
  • Establish and maintain effective working relationships with internal Neighborcare departments and external referral sources, community partners, and key stakeholders
  • Collaborate with clinical leaders to sustain and improve professional clinical activities and structures. Identify gaps in knowledge, skills, or practice and works with clinical leaders to create and implement plans to address.
  • Participate in evaluation of clinical quality data and application of evidence-based practices to improve clinical practice, patient outcomes, and patient experience data to develop, implement, standardize and/or improve systems that affect and/or enhance diabetes and nutrition care services system wide.
  • Perform the duties of the RDN (if applicable) and CDCES job descriptions as required during scheduled clinic hours.

Required for this job:

  • Knowledge and expertise in systems level DSMES and MNT Program management, health care team integration, and employee professional development.
  • Knowledge and expertise in quality improvement principles, chronic care model, case management, patient education techniques and health behavior change strategies.
  • Expertise in mentoring co-workers desiring MNT and DSMES knowledge and skills improvement.
  • Proficient in using electronic database and reports programs, expertise in oral, written and interpersonal communication.
  • Knowledge and understanding of mental illness and chemical dependency and its impact on patient self-care.
  • Ability to communicate patient care skills effectively to patients from a variety of backgrounds, cultures, socioeconomic levels and with varying levels of health literacy and English proficiency.
  • Ability to meet and comply with HIPAA/Confidentiality policies and procedures and to handle confidential and sensitive patient and staff information.
  • Ability to effectively perform job duties balancing demands of a busy clinic with a variety of conditions, and time constraints, and resources, while managing administrative schedules.
  • Ability to self-assess knowledge and skills, and continuously develop clinical expertise through Neighborcare Health and community-based continuing education.

EDUCATION

Required: Bachelor’s degree in dietetics or related field.

Preferred: Master’s degree in nutrition or related field.

LICENSES/ CERTIFICATIONS/ REGISTRATIONS

Required: Valid Washington State Registered Nurse or Registered Dietitian license. Valid and active Certified Diabetes Educator credentials through the NCBDE. Current CPR certification.

Years of Relevant Experience Required:

2 years RDN, CDCES or RN, CDCES experience in a primary care, hospital and/or home care setting or comparable social service setting.

Years of Relevant Experience Preferred:

1 years of employee management or ADA or ADCES DSMES Program management experience

Compensation 

The wage range for this position is minimum $76,107.20 to $105,096.00 maximum.

Compensation will be determined based on years of relevant experience.
