We are a non-profit organization dedicated to supporting Individuals with Developmental Disabilities, by providing Vocational and Community Living Services. We have been serving Individuals with Developmental Disabilities in San Luis Obispo County and Monterey County since 1980. Our goals is to promote independence and inclusion for adults with intellectual and/or physical disabilities through vocational training, education and adaptive living skills programs, advocacy, and community living services. Â
The Vocational Case Coordinator is responsible for assigned case management provided in the vocational aspect including Internships of Individuals'Â served. This position has supervisory duties.
Essential Duties Include:Â Â
Oversee Individual’s assessment to determine their goals, preferences and needs and provide suggestions to assure meeting of these needs. Ensure contact customer satisfaction, by assessing the effectiveness of the placement of Individual’s served.
Work with Individual’s served in a proactive manner to encourage behavior changes as needed and encourage growth.
Provide individuals with continuous instructions for work specific to vocational growth and training objectives that guarantee and correspond with abilities and development.
Understand and submit all paperwork as required by deadline including transportation paperwork, attendance, job coach notes, objective sheets, incident/accident reports, time studies and case management documentation.
Collaborate with management in addressing worksite challenges, as well as, assessing potential crew opportunities.
Execute proper allocation of agency resources (product, space, time, people, etc.).
Ensure that staff has accurate information regarding Individual’s served and in a timely manner.
 Ensure that staff is trained on how to complete data charts and all required goal tracking paperwork. Supply quarterly updates.
Education / Experience:Â
A high school education or equivalent is required. Previous experience working with Individuals with Developmental Disabilities is preferred, but not required.Â
Qualifying Specifications:Â
Must have own vehicle/reliable transportation Â
Must maintain a current California driver’s license.Â
Must provide on-going proof of current state required auto insurance.Â
Must have an acceptable driving record as determined by criteria established by the agency’s insurance carrier.Â
Must clear fingerprints through the California Department of Justice and the FBI.Â
Must successfully complete First Aid / CPR training (Training is offered onsite)Â
Physical Requirements:Â
Essential functions of this position require that employees regularly sit, stand, bend, twist and walk.Â
Employees may occasionally be required to lift or move up to 50 lbs.Â
