Job Description Summary

The MUSC Care Transitions (CT) Program Manager reports to the System Manager of Care Transitions Management and closely collaborates with Population Health, inpatient and outpatient care teams, Primary Care and Post-Acute Care teams and leadership. The CT Program Manager supports departmental, ICCE, and Medical Center goals. Goals include achievement of clinical and quality measures, operational efficiency, and fiscal targets, with a focus on reducing hospital readmissions by providing support services to patients. The position is responsible for coordinating and facilitating activities and services across the system regarding the patient care continuum and includes patient outreach and ongoing patient care coordination support. The CT Program Manager will work in collaboration with inpatient and outpatient nursing, case management, social work, physicians, outpatient clinic offices, home health and other agencies that provide care across the continuum to improve patient outcomes and reduce readmissions. The CT Program Manager contributes to the improvement in the quality of life for patients, and the reduction of expenditures for the care of disease-related complications and/or social/environmental needs, by focusing on the coordinated management of patients at high risk for inpatient hospital readmissions and emergency department utilization. The CT Program Manager effectively communicates with health care providers and community resources to address and support social determinants of health (SDOH) needs and ensures the provision of patient education and support processes that improve health outcomes, reduce patient hospitalizations, improved patient morbidity and mortality, and lower the use of inpatient and outpatient healthcare resources.

Job Description

Required Minimum Training and Education: Bachelor’s degree in Nursing, Social Work or other related field preferred. Master’s degree in Nursing, Social work, or related field strongly preferred. Three (3) years of related work experience as a registered nurse or social worker required. Case Management or social care coordination experience preferred. Ensure the provision of care transition services in collaboration with other health professionals. Current knowledge base of procedures, techniques, equipment, technical advances and regulatory agencies and standards within the field of care coordination. Requires excellent oral and written communication skills, strong organizational and time management skills. Ability to establish cooperative relationships and must be sensitive to the needs, concerns and safety of patients and visitors/families of diverse cultural orientation.

Required Licensure, Certifications, Registrations: Current South Carolina licensure or licensure in a compact state as a registered nurse, social worker, or case manager is required for Nurse applicants. Current Basic Life Support (BLS), either a certification from an American Heart Association (AHA) BLS for Healthcare Providers (or AHA recognized equivalent) or an American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider is required.
Physical Requirements: Continuous requirements are to perform job functions while standing, walking, and sitting. Ability to bend at the waist, kneel, climb stairs, reach in all directions, fully use both hands and legs, possess good finger dexterity, perform repetitive motions with hands/wrists/elbows and shoulders, reach in all directions. Maintain 20/40 vision corrected, see and recognize objects close at hand and at a distance, work in a latex safe environment and work indoors. Infrequently lift and/or carry objects weighing 10 lbs ( /-) unassisted.
