Parent Child and Family Health (PCFH) is a unit within the Public Health Division of the Multnomah County Health Department with multiple home visiting programs. These programs use community-based strategies to ensure that diverse populations in Multnomah County have access to services that best serve their communities. PCFH programs ensure a trauma-informed, equity focused systems approach to Multnomah County home visiting resources. The primary goals of PCFH programs are to improve pregnancy outcomes, decrease health inequities among people of color, improve child health and development, and improve economic self-sufficiency of the family through the interruption of toxic stress and build resiliency.
The PCFH Community Health Nurse (CHN) position will provide community health nursing services and consultation to the Oregon Children Development Coalition (OCDC) staff for children up to 5 years of age, as well as Asthma Home visiting nursing services as part of the Healthy Homes Asthma Home Visiting (HHAHV) program. The nurse home visitor is responsible for maintaining the highest standards in clinical nursing practice and adherence to policies and procedures of Multnomah County Health Department. This position also requires the ability to speak, write and read fluently in the Spanish and English languages.
As the Community Health Nurse (CHN) , you will:
  • Provide child health assessments, counseling and teaching and other interventions, links to health care and other resources
  • Develop plans to meet health goals of individuals and families through home visits and group activities in the community
  • Case manage and coordinate services with other agencies and health care providers to provide for the psychological, and social service needs of assigned individuals and families
  • Conduct selected health and developmental screenings for clients and their children
  • Assist in developing health related protocols and assessment of health practices in the classroom setting
  • Knowledge of Asthma case management, including protocols, medications, interventions, and teaching for families and children
  • Give health related presentations to OCDC staff
  • Participate in regular team meetings, other health department, county and community meetings, and in professional development activities

TO QUALIFY:

Required Minimum Qualifications/Transferable Skills*:
  • An Associate's degree in nursing from an accredited nursing program AND a license to practice as a registered nurse as outlined by Oregon Revised Statutes (ORS) 678.021. (A Bachelor's of Science in Nursing (BSN) is preferred.)
  • A current BLS CPR certification is required.
Other:
  • This position requires the ability to speak, write and read fluently in the Spanish and English languages. Language testing is required.
  • Must have a valid State issued Driver's License.
  • Must be able to Pass a background check through Central Background Registry (CBR) and OCDC.

  • Experience or ability to synthesize data in order to independently solve problems or formulate recommendations
  • Deep understanding of and experience addressing the systemic barriers that perpetuate disparities for community members who experience marginalization

COVID-19 Vaccination Requirement: To protect the health of the community and employees, Multnomah County requires employees to be fully vaccinated against COVID-19 or have an approved medical or religious exception as a qualification of employment.
