RESPONSIBILITIES:

Interviews clients, family members, and/or referral source to obtain information about the immediate problemand the client's rescources and ability to deal with the problem.

- Completes and submits comprehensive in-home assessment of the consumer using the state - approved assessment tool with the consumer and/or caregiver

Identify informal/formal supports currently in place with the client, idenitfy and consider the consumers values and preferences, and identify potenial service options available to them from both publically and privately funded programs.

Follow up with individuals and families following the assessment to provide any additional information needed.

This not all encompassing and other duties may apply as assigned
