Responsibilities:
· Participating in the development, implementation, and ongoing review of the plan of care.
· Completing a case-management progress note corresponding with each on-site visit or encounter and upon notable change.
· Coordinate and monitor services to ensure client needs are met.
· Collaborate with other professionals and community resources to support clients including referrals for other services.
· Coordinate with Program Director and Nurse to ensure participant satisfaction.
· Other functions as directed.
Requirements:
· Bachelor's degree
· Ability to speak both Spanish and English required
· Effective communication and interpersonal skills
· Ability to work independently and as part of a team
· Excellent organizational and time management skills
· Ability to remain focused & productive each day without the supervision of a supervisor.
· Valid driver's license and reliable transportation
