We are looking for a Wound Care Nurse (RN) to join an SNF in Hempstead, NY.
*Position Overview:*
As a Wound Care Nurse, you will play a crucial role in the well-being and recovery of our patients. You will be responsible for assessing, planning, implementing, and evaluating the wound care needs of patients under your care. The ideal candidate will have a strong background in wound management, excellent clinical skills, and a passion for providing high-quality patient care.
*Key Responsibilities:*
* Conduct thorough wound assessments and collaborate with the healthcare team to develop individualized care plans.
* Administer and monitor advanced wound care treatments, including but not limited to wound vacs, dressings, and debridement procedures.
* Educate patients and their families on proper wound care techniques and prevention strategies.
* Collaborate with physicians, surgeons, and other healthcare professionals to ensure coordinated and effective patient care.
* Maintain accurate and up-to-date patient records, documenting care provided and progress made.
* Stay informed about the latest advancements in wound care and incorporate evidence-based practices into daily routines.
*Qualifications:*
* Current Registered Nurse (RN) license in NY.
* Certification in Wound, Ostomy, and Continence Nursing (WOCN) is highly desirable.
* Minimum of 1 year of experience as a Wound Care Nurse in a healthcare setting.
* Strong clinical assessment and critical thinking skills.
* Excellent communication and interpersonal skills.
* Ability to work collaboratively in a team environment.
