AURORA Home Health is seeking an experienced Home Health Registered Nurse for field visits in DALLAS, TX area. Ideal candidate will have a year or more of recent home health experience. Must have clinical assessment skills and knowledge of OASIS.

About the Position:

Registered Nurse are responsible for comprehensive assessments in the patient's home, including the completion of OASIS using electronic documentation (Axxess).

• Evaluate patient and home health environment to determine required services and involve the patient and patient's family in developing the plan of care.
• Develop the POC based on findings from evaluation visit.
• Serve as the patient's advocate and maintain ongoing communication with physician, referral sources, caregivers and facility management to facilitate coordination of care.
• Administer medications and treatments as prescribed by physician
• Provide direct supervision of nursing clinical care team members (LVN's & HHA's).
• Participate in regular case conference and in-services

Requirements

• One year of previous adult home health experience required
• Current Texas Registered Nurse (RN) license
• One year of clinical nursing experience
• Current Texas driver's license
• CPR Required
• Experience with healthcare software, Axxess is a plus
• One year of previous OASIS experience

Pay: $80,609.00 - $83,123.00 per year
