Responsibilities

Primary Care-Senior Services in the Austin/Albert Lea region is seeking a Nurse Practitioner or Physician Assistant to work as part of a multidisciplinary care team to provide care to those living within our community through the Senior Services Skilled Nursing Facility (SNF) practice. Although this is a highly autonomous and independent practice, the NP/PA will work collaboratively with internal medicine physicians, registered nurses and others to provide holistic care. This care includes acute, chronic, preventive, and palliative care services to patients in SNFs (post-acute and long-term care units) in the SEMN MCHS region, with the primary location of Austin/Albert Lea. Provision of care includes but is not limited to comprehensive evaluation, ordering, and interpreting diagnostic tests, prescription of pharmacologic agents and treatments and non-pharmacologic therapies, and counseling patients. Ideal candidates have experience in Palliative Care, Primary care and/or Geriatrics. Travel is required.

Qualifications

Nurse Practitioner: Minimum of Masters preferred Doctoral Degree graduate of an accredited school of nursing and Nurse Practitioner program. Current Minnesota RN license and certification as an ACNP, ANP, GNP or FNP.

Physician Assistant: Graduate of accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the Minnesota State Board of Medical Practice.

Candidate must be eligible for DEA registration.

Additional Qualifications:

Minimum of 1 year of experience in a medical clinic or hospital is preferred. Strong written and oral communication skills are required, and candidate must have demonstrated skill in working collaboratively with all members of the healthcare team. Experience in geriatrics, primary care or internal medicine preferred.
