As a Hospice Nurse, you will provide specialized care and support to patients and their families during end-of-life stages.

Your role as a nurse are the following

Responsibilities:

• - Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs
• - Develop and implement individualized care plans based on assessment findings - Administer medications and treatments as prescribed by the physician
• - Monitor patients' condition and report any changes to the interdisciplinary team - Provide emotional support and counseling to patients and their families
• - Educate patients and families on disease management, symptom control, and end-of-life care options
• - Collaborate with other healthcare professionals to ensure holistic patient care Strong communication and interpersonal skills to effectively interact with patients, families, and the interdisciplinary team
• - Ability to provide compassionate care while maintaining professional boundaries
• - Proficient in using electronic medical record systems such as HOSPICE MD
