A HEART OF HOPE LLC is Hiring a Direct Support Professional (DSP) Near Chesterfield, VA

A Heart Of Hope LLC is seeking Full-Time, Part-Time, and PRN Direct Support Professionals (DSP) to join our team. The energetic candidate will work in a lively environment supporting adults with developmental disabilities. Skills needed include professionalism, compassion, and the ability to adapt easily to a changing daily schedule, work well in a team and demonstrate creative problem-solving skills. The DSP will work closely with the residents and manager of the home to plan daily activities to include transportation to work, day programs, and medical appointments during the week and participation in preferred activities of the resident.

The DSP will also support the resident with activities of daily living such as dressing, showering and hygiene, cooking and cleaning while ensuring their safety and maximizing their physical, emotional, intellectual and spiritual abilities.

Job Responsibilities

1. Provides day-to-day organization and implementation of activities that promote an organized and positive environment. Ensures as the primary professional working with individuals, an environment that is person centered, organized, non-reactive, and promotes independence.

2. Is able to independently make decisions in regards to best practices, quality assurance, risk management and problem solving. This includes timely response to and documentation and reporting of serious incidents.

3. In partnership with supervisor, has working knowledge of 12VAC35-105; 12VAC35-115; and 12VAC35-122. This includes promoting rights, reporting serious incidents and general compliance with all regulations, Policies/Procedures, and HTFS Employee Handbook.

4. Is able to model and coordinate person-centered, least restrictive and community-integrated philosophies and supports.

5. Participates in an active circle of support, including the individual, AR, family members, and staff that work closest with the individual to collaborate and advocate annually and as needed to develop independence and services in the least restrictive environment. Ensuring communication as needed within the circle of support and with supervisor.

6. Completion of required documentation including progress of measurable goals and methods to achieve goals, completion of quarterly reviews as assigned by manager, required daily/weekly/monthly progress notes and accurate support services records.

7. Carries out duties as assigned to ensure the environment is clean, safe, and maintained in good condition. Reports maintenance needs in a timely manner.

8. Participate in an environment that promotes health and safety through medication administration assistance, Medication administration audits, attendance of needed medical appointments, lab work, and well visits as assigned by Manager; assisting with, attending medical supports; documentation of changes in medical condition and needed supports; and communication with supervisors regarding any medical needs or services rendered.

9. Applies skills learned during medication administration training. This includes, but is not limited to, checking in medications, privacy during medication administration, a process free of medication errors, and completion of related documentation.

10. Demonstrates working knowledge of and complies with policy on Handling Participant’s Funds and Business Office Procedures.

11. Demonstrates a working knowledge of, complies with and models policy on A Heart Of Hope LLC System to Prevent Abuse, Neglect, and Exploitation. Complies with HTFS DDM Code of Ethics. This includes reporting any known, suspected, or witnessed abuse to the Manager, other leadership per policy immediately and as a Mandated Reporter.

12. Demonstrates the ability to coach, teach and mentor individuals in all aspects of home management and personal independence. This includes actively teaching and promoting individual rights and self-advocacy.

13. Attend at least monthly meetings with all staff to ensure consistent and pertinent communication regarding services and supports provided.

14. Responsible for identifying and completing all A Heart Of Hope required trainings.

15. Responsible for maintaining work schedule, including arrival to work on time, notification to supervisor of absences at least two hours ahead of scheduled work shift, coordinating with fellow team members for coverage and supervisor as needed. Consistent attendance is required. Monitors and manages PTO usage and overtime. Documents time worked accurately.

16. Acknowledges and commits to providing supervision as identified. This includes understanding and recognizing that leaving individuals without supervision rises to the definition of neglect. Recognition while in this position requires coordination of coverage with supervisor in cases of emergency, natural disasters, staff shortages, or other circumstances in which leaving the location is not possible.

17. Carries out responsibilities and duties in a manner that is professional, timely, effective and efficient, including other duties as assigned.
