Job Description
At the direction of the Medical Director, the Full- Time Psychiatric Mental Health Nurse Practitioner works collaboratively with PACE Physicians and team providing assessment, planning, primary care, and continuing medical education to patients.

Skills and Certifications:
Certified as a Psychiatric Mental Health Nurse Practitioner
Licensed in California, State CDS Certification DEA, and NPI License.
2-5 years experience
Psychiatric Mental Health Nurse Practitioner Job Duties:
Provide psychiatric consultation, psychiatric illness evaluation, counseling, and treatment for participants in collaboration with PACE Physicians and Community-Based PACE Physicians.
Perform in-person or telehealth comprehensive psychiatric assessments on participants in collaboration with a physician and develop a comprehensive care plan for each participant.
Collaborate with physician(s) in discharge planning from acute or long-term care settings.
Oversee care of patients in the nursing home, collaborating with the PACE Physician and/or Community-Based PACE Physician, and making regular visits as needed.
