Your role:
Pen Bay Family Medicine is located at the Beebe Health Center in Rockport, Maine. Under the direction of the Clinical Care Manager, the RN Patient Care Coordinator uses professional nursing skills to coordinate the health care needs of patients in the practice identified as at risk due to chronic or acute illnesses or those in need of preventative health services. The RN Patient Care Coordinator works with the provider, practice care team, patient and family to promote self-care management and provides direct care appropriate to the role of the nurse including patient education, motivational interviewing and development of self-care action plans.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

• • Education: N/A
  • License/Certifications: Licensed RN by Maine State Board of Nursing.
  • Experience: Minimum of 2 years' clinical experience preferred. Physician practice or similar ambulatory clinic setting preferred.
  • Strong assessment, goal setting, problem-solving, and decision-making skills.
  • Ability to establish effective working relationships with patients and care team members.
  • Ability to self-motivate, manage conflict and adapt to change essential.
  • Ability to independently follow established policies, procedures, and care plans.
  • Other Certification : As required by department, including Annual Electronic Fetal Monitoring competency for any RN assisting Laboring Women
