TR2 Please ensure you read the below overview and requirements for this employment opportunity completely. $36 - $44 per hour $2,800 Sign-on Bonus $2 hour NOC Shift Differential $3 hour Weekend Shift Differential (Day NOC) Part-time and On-Call 12 hour shifts Day Saturday and NOC Friday JOB SUMMARY:: A Licensed Practical Nurse (LPN) working in our Skilled Nursing facility is responsible for the daily nursing care needs of our residents. This position is a valuable clinical team member contributing to the smooth, organized and efficient operations of the nursing department. RESPONSIBILITIES: * Provide primary nursing care to residents with varying backgrounds and medical histories
* Assists with new admissions * A Licensed Practical Nurse (LPN) will monitor and communicate changes of condition to provider * Provide medical intervention as needed for emergent needs * Communicate needs of resident to outside providers including but not limited to: Primary Care Provider, Home Health Agency, Pharmacy, etc. * As a Licensed Practical Nurse (LPN) you would communicate resident condition to family, and or responsible party members * Implement and follow Primary Care Providers orders * Other Duties as Assigned REQUIREMENTS: Licensed Practical Nurse (LPN) Licensure in the State or practice BENEFITS: Benefits of being a Sapphire Health Services employee include, but is not limited to: * 401K
